The Cycle of Engagement Well Visit Planner (COE WVP) Approach to Care is a model for engaging caregivers of children under age 6 and their child’s care teams in an ongoing, collaborative partnership.
As part of the Cycle of Engagement Model, the COE WVP Approach to Care includes three interconnected tools to engage and partner with families and improve child health. These include:

We are driven by our dedication to partner with families and care teams to meet the great need and possibilities to promote the early and lifelong health of children, families, and communities. The COE WVP Approach to Care is the product of over 30 years of partnership with families, providers, and experts at the national, state, health plan, practice, and provider. With initial support from The Commonwealth Fund and the Health Resources and Services Administration, we have worked in close collaboration with Family Voices and leaders of the American Academy of Pediatrics’ Bright Futures Guidelines for Health Supervision of Infants, Children and Adolescents to translate guidelines into family-friendly, actionable tools to customize and improve the quality of well child care services based on family reported assessments and priorities. Current funding comes from a grant from the Robert Wood Johnson Foundation.
